Instructions for completing the Funds Request Form:Name: Name of the coach or other Central Lake Public Schools employee requesting funds.
Date: Date the form was filled out. Sport/Team: Identify the Sport or Team for which the request is being made. Who Would Benefit from This: Identify the individual(s) who will benefit from granting the request. Request: Describe the item to be purchased and the amount that you are requesting from the Boosters Club. Documents like catalog pages, quotes or invoices are very helpful and should be attached if available. Other Sources of Funding: Please identify any other source(s) of funds that could contribute to this purchase. For example, if the CLPS athletic account has funds that could be applied to this purchase, indicate that fact here. Price of Items: Total price of the item. If the item costs more than the amount being requested we still ask that the full price be supplied on this line. Tax & Shipping: Indicate any additional amount required to acquire the item. If there is an installation or other charge required to place the item in service please include that as well. Total Cost: The sum of Price-of-items and Tax-&-Shipping. The remainder of the form should be left blank. The Athletic Director should sign and date the form if he/she finds the request to be reasonable. Once signed, the Athletic Director may submit the form to the Secretary of the Central Lake Boosters Club for consideration at the next Directors' meeting. A club director will sign the form indicating whether the request was Approved or Not Approved. The Board of Directors meets approximately once per month during the school year. Forms should be submitted to the Athletic Director no later than six (6) weeks prior to the start of the season from which the request is made. |
